Certified copy of Form 24
Certified copy of Form 49
Extract/Certified copy of the resolution of the Board of Directors
Photocopy of Certificate to commence business (*only for Public Listed Co./Berhad)
Photocopy of Identity Card/Passports of all the authorized signatories
Company Rubber Stamp.

For Sole Proprietor and Partnership
Photocopy of Certificate of Business Registration
Certified copy of Business Registration Certificate
Trading License (Borang A/Borang B/Borang I)
Photocopy of Identity Card/Passports of owner (Sole-prop) or all partners (partnership)
Letter of Declaration (partnership)
Company Rubber Stamp.
